According to TSS, this box art is appearing all over Amazon.  Looks official, but who knows?  I know I like it.

Looks photoshopped. The similarity to a S&M image has me skeptical.

There are far too many obvious differences for it to be Photoshopped, it's just a similar pose. Hand, shoe, quills, body, lighting, camera angle, everything's different enough that it would be basically impossible for it to be convincing (far too difficult for it to be worth the effort).

It's real, just a bad design. However, "real" hopefully doesn't mean "final".

This is probably fake. It's the exact same pose  as the Mario and Sonic at the Olympic games boxart, and a good photoshopper could do that easily.

You people preaching Photoshop totally fail. Sonic in the boxart is perfectly aligned with the viewer while the M&S artwork is angled about 10-15 degrees. Not to mention that shoe is completely different. And if some random dude really did photoshop this, then where did the werehog art come from?

That being said, I have no clue what the rest of you were expecting in this boxart. Sonic 06-like Sonic-running-through-fields? At least this boxart is honest about what the game is all about. That alone deserves at least a little respect. Unless of course, the people who said this game is 99% playing as Sonic in one way or the other are totally lying to us, in which case, >:O.
